Adoption Form?

I have recently "adopted" a dog that was going to be euthanized for no reason. I took him home to do a quick assessment before dropping him off at his forever home, but they have not formally adopted him yet, as the man who will be taking the dog is away right now, and will be home this weekend.

For those of you in rescue, what kind of form do you have new owners fill out? Would anyone be willing to send me an example, so I can change the wording? Or even some advice on how to make the form would be very much appreciated.

The adoption papers were signed today :D :D :D

Koda is doing amazing in his new home. He is very attentive to his new daddy, and he never stops smiling or wagging his tail. So far, there have been absolutely no behavioural problems, so fingers crossed it will stay that way. If it changes, I know they will call me, and we can work it out. They are amazing people, and I am so happy that we found them for Koda.

I also got some thank you hugs from Koda while we were visiting. This is one story that ended very, very happily <3
