I think that you already know that the pooping issue is directly linked to the toilet training. He's telling you he's uncomfortable with the balancing over the toilet seat to poop. That may not be fixable even by going back a step. Not all cats will toilet train.
The peeing randomly issue was there before the toilet training and really should have been resolved before you attempted the toilet thing. Given it happened before, it probably is a separate issue with different motivation. When you say he pees in your office, where in the office? Is it always in the same place? What have you been cleaning up with? Are the pillows down at all? What did you wash them with?
