Dooley still challenged

I would ask for a copy of the biopsy report, you paid for it thus you are entitled to it. If He does have to have surgery again, I read somewhere that instead of a cone of shame, dress the cat in baby clothes, t shirt or onesie size 3 mo or 6 mo. If onesie you might have to try it on, Mark where a hole needs to be cut and experiment a little. Saying prayers for a good outcome.

We did speak of the biopsy, although the conversation was more focused on treatment moving forward.

I did order Denamarin and will give it a go. The Doctor asked me to stay away from the chewable version so we aren't getting things other than the product into his system in advance of his Meds.

Anyway, I'll post more soon and Dooleycontinues to do well with his current program.

Hi Paige,

The biopsy results are in, the doctor's report is tentative. I don't have a copy of either yet. My guess is I will have them either Friday or Saturday. The Doctor keeps pretty brief office hours, Monday, Friday, Saturday although she is at the hospital to the tune of 80+ hours a week. Her first day back from vacation was the day Dooley had his post surgery follow up appointment.

Obviously none of this can go as fast as I'd like and there are plenty of other patients with critical needs as well.

Here's the biopsy report, had it all along just didn't know it was contained in a larger document.

PROCEDURES/LABORATORY TESTS:

Histopathology of left lateral and caudate process of caudate liver lobe:

A, B: Left lateral and caudate lobes, single wedge biopsy obtained from each site, both evaluated, both demonstrating consistent arteriolar hyperplasia of small to medium portal tributaries, and occasional arterioles are noted randomly within intervening lobular tissue. There are increased numbers of portal tributaries as compared to the volume of tissue. In some of the portal regions, there is an absence of convincing portal veins and dilated large to medium portal arteries. Ductular reaction is minor. Central veins are dilated though otherwise typical. Sinusoids are frequently ectatic. Vacuolar degeneration is minor. There are mildly increased lipogranuloma.

There is no evidence of fibrosis, necrosis or inflammation.

Interpretation: Insufficient portal blood supply (both lobes)

COMMENTS: These well procured biopsies display characteristic changes expected with insufficient portal blood supply, regardless of cause. Age and severity of clinical signs suggest congenital portosystemic shunting (presumably intrahepatic since surgery negative for extrahepatic shunt), or severe primary hypoplasia of intrahepatic and extrahepatic segments of portal vein (rare but does occur in cats). Correlation with sonography is encouraged to differentiate, the latter expected to be accompanied by sonographic evidence of evidence of parenchymal disease as cause of intrahepatic acquired portal hypertension in these biopsies

PATHOLOGIST: TAMMY JOHNSON, DVM, Diplomate, American College of Veterinary Pathologists

I also mistakenly mentioned the IM Doctors colleague at Cornell who she was conferring with was Dr Marnin Forman where this report states the contact as Dr Sharon Center.

Dooley is doing pretty well, just wish I could dial back the Keppra so he wouldn't be so irritable.
