PROCEDURES/LABORATORY TESTS:
Histopathology of left lateral and caudate process of caudate liver lobe:
A, B: Left lateral and caudate lobes, single wedge biopsy obtained from each site, both evaluated, both demonstrating consistent arteriolar hyperplasia of small to medium portal tributaries, and occasional arterioles are noted randomly within intervening lobular tissue. There are increased numbers of portal tributaries as compared to the volume of tissue. In some of the portal regions, there is an absence of convincing portal veins and dilated large to medium portal arteries. Ductular reaction is minor. Central veins are dilated though otherwise typical. Sinusoids are frequently ectatic. Vacuolar degeneration is minor. There are mildly increased lipogranuloma.
There is no evidence of fibrosis, necrosis or inflammation.
Interpretation: Insufficient portal blood supply (both lobes)
COMMENTS: These well procured biopsies display characteristic changes expected with insufficient portal blood supply, regardless of cause. Age and severity of clinical signs suggest congenital portosystemic shunting (presumably intrahepatic since surgery negative for extrahepatic shunt), or severe primary hypoplasia of intrahepatic and extrahepatic segments of portal vein (rare but does occur in cats). Correlation with sonography is encouraged to differentiate, the latter expected to be accompanied by sonographic evidence of evidence of parenchymal disease as cause of intrahepatic acquired portal hypertension in these biopsies
