If there are outside sutures then yes the potential for them to pick at it exists the time it takes for the suture line to heal up... luckily my vet doesn't leave any external sutures. I find they heal so fast, within days rather than weeks.
My vet rarely tells me to "keep them quiet" or limit their exercise...because I will burst out laughing and he will join when he realizes who he said that to... he knows the Savannahs are high energy and there simply is no way to keep them completely calm and non-active. Either your vet doesn't understand or they are attempting to cover themselves in case your cat is crazy active and reopens her suture line somehow.
