I am sorry to hear about your vet's reaction to Vivi.
To speak to your point about putting paws with claws out through the grate in a cage- Diablo, my F5 totally misnamed angel (not fallen angel) of a cat will do this. He isn't trying to scratch, he just hates being in a cage. If he can get close enough to grab, he grabs claws in with his paw, wrapping the toes around your fingers to get a good grip. He actually does the same thing when he is really happy, he grabs with a paw flared, claws out for a hand to snuggle with- he never scratches, but he likes to get good grip on me.
